What Is Support Coordination?
Support Coordination is an NDIS-funded Capacity Building support that helps you understand and implement your NDIS plan. A support coordinator works with you to identify your goals, find and connect with service providers, build your capacity to manage your own supports, and resolve any issues that arise. There are three levels of support coordination: Support Connection, Support Coordination, and Specialist Support Coordination. At Ubuntu Support Services, we primarily provide Level 2 (Support Coordination) and Level 3 (Specialist Support Coordination) services.

Our support coordinators are experienced, qualified professionals who genuinely care about helping you achieve your goals. We take the time to understand your situation, your aspirations, and your challenges. We then work with you to develop a plan of action that connects you with the right services and supports. This may include researching and recommending service providers, helping you understand service agreements, coordinating between multiple providers, supporting you through plan reviews, advocating on your behalf when needed, and building your skills to self-coordinate over time.
